3. Is any Proposed Insured currently receiving treatment, ever had treatment, or been advised to have treatment for internal cancer ( including skin), lymphoma, leukemia, or melanoma?
4. Within the past 5 years, has any Proposed Insured had persistenly abnormal PSA readings, or abnormal mammogram, pap smear, colonoscopy, or other cancer screening test where cancer has not been ruled out?
